Nurse Care Coordinators utilize their skills of assessment, along with their knowledge of disease processes, and normal body function to help get their
clients the best medical care possible. They assist in determining the appropriate medical professional to provide the care you need.
Once your physician determines the care and services you need your Medical Care Coordinator helps expedite those services by getting treatments started and intervening with your insurance or payer source as may be needed. The Medical Care Coordinator can also make home visits and make arrangements for home care when needed. Educating you, your loved ones, in understanding your diagnosis in terms you can understand is an essential part of her job.
Communication on your behalf to your medical providers is an essential part your Medical Care coordinator’s responsibility. She will make sure your physicians hear your questions and symptoms. She will also provide status reports to all your physicians.
